
What Is Fish Oil, What Does It Do?
A bottle of fish oil may sit quietly beside your vitamins for years while never being asked a rather important question: What exactly do you do for my health?
Omega-3 fatty acids are biologically important. But they are neither a miracle nor a meaningless marketing manipulation. This edition will reveal what EPA and DHA do, where their benefits are most credible, and how to decide whether fish oil belongs in your health and wellness plan.

Wisdom is seeing all of life from God’s perspective. In this specific case, it begins before the purchase. It asks what a product is, what it does, whether it is needed, and how it fits into your overall plan of faithful stewardship. We do not honor God by believing every supplement promise, nor by dismissing useful science because marketing has exaggerated it. We honor Him by seeking understanding, acquiring wise counsel, testing claims carefully, and caring for the body He designed without fear or obsession.
That pursuit of understanding for us begins by revealing what omega-3s actually do.

Practical Points to Help You Think Clearly About Fish Oil.
1. Many people take fish oil without knowing why.
Fish oil has been promoted for the heart, brain, eyes, joints, mood, pregnancy, inflammation, and longevity. That is an impressive résumé for one softgel.
The problem for the consumer is that different claims have different levels of evidence, different dosing requirements, and different safety considerations. A person taking a modest over-the-counter product for general wellness should not follow the same plan as someone receiving a prescription omega-3 for very high triglycerides.
Before deciding whether fish oil is useful, we need to understand its active ingredients.
2. Omega-3s help build the membranes surrounding your cells.
Imagine every cell in your body as a home. Its outer membrane acts like a living wall. It helps regulate what enters, what leaves, how signals are received, and how the cell responds to hormones, stress, inflammation, and neighboring cells.
Those membranes are built partly from fats.
That means the quality and balance of fats available to your body influence the building materials used in these active cellular walls. Fish-derived omega-3s, specifically, EPA and DHA become part of the structure and communication systems your cells depend upon.
3. ALA, EPA, and DHA are related, but they are not identical.
ALA, or alpha-linolenic acid, is the plant-based omega-3 found in flaxseed, chia seeds, walnuts, and several plant oils. These are valuable whole foods, and I regularly encourage their use when tolerated. But, your body must convert ALA into the longer-chain omega-3s EPA and DHA. That conversion is inefficient and limited.
Think of ALA as raw timber. It has value, but it must be processed before it can become finished useable lumber. EPA and DHA are more like building materials ready for use.
EPA and DHA come primarily from fatty fish, fish oil, krill oil, and algae-derived products. If your goal is to meaningfully raise EPA and DHA intake, direct dietary or supplemental sources are more reliable than depending on ALA conversion alone.
4. DHA is especially important for structure; EPA often contributes more to signaling.
DHA is a major structural fat in the brain and retina. It helps support the fluid membranes required for nerve-cell communication and is especially relevant during pregnancy, breastfeeding, and early neurodevelopment.
EPA is often emphasized for triglyceride metabolism, inflammatory signaling, and adjunctive mood support.
This distinction is useful, but not absolute. EPA and DHA overlap and work together. A balanced formulation is reasonable for general support, while a goal-specific plan may favor more density of one over the other.
The important lesson is that “fish oil” is not one uniform substance. The EPA-to-DHA balance may matter depending on the goal you are trying to accomplish.
5. Omega-3s do not simply shut inflammation off. They help the body resolve it.
Inflammation is not inherently bad. You need it to fight infection, heal injuries, and repair tissue. The problem arises when the emergency tissue response inflammation invokes remains active longer than necessary.
Think of inflammation like a fire department. When a fire begins, you want the firefighters to arrive. But if they never leave—continuing to spray water, break windows, and tear open walls—the prolonged response creates unhelpful damage of its own.
EPA and DHA provide raw material for compounds involved in inflammatory resolution, including resolvins, protectins, and maresins. These signaling molecules help move the body from alarm toward cleanup, repair, and restoration.
So “fish oil eliminates inflammation” is too simplistic a statement.
A more accurate statement is that omega-3s support the body’s orderly resolution of inflammation, modulating its intensity and duration.
6. The strongest conventional use is in triglyceride reduction, which is not the same as universal cardiac protection.
Omega-3s can lower triglycerides, especially at prescription-level doses used under medical supervision. That is a meaningful benefit.
But lowering triglycerides is not the same as stating in marketing material that over-the-counter fish oil prevents heart attacks in every adult. Cardiovascular trial results vary according to the formulation, dose, population, and health context.
For most people, the best starting recommendation is to eat fatty fish regularly, twice a week, as part of a healthy natural whole food dietary pattern.
Fish oil may help fill an existing dietary gap, but it should not be considered as your guaranteed cardiovascular insurance policy in a capsule.
7. Brain, mood, and joint benefits require realistic expectations.
DHA is essential to brain structure, yes, but it is not the only factor. Brain health is incredibly complex and influenced by sleep, blood pressure, glucose control, movement, social connection, nutrition, stress, and many other factors.
EPA-rich formulations may provide adjunctive support for some with depressive symptoms, but can not stand alone or replace counseling, prescribed treatment, sleep, movement, prayer, community, and appropriate medical care.
Omega-3s may also help inflammatory joint conditions, I have found them to be quite helpful in this context. However, they cannot rebuild damaged cartilage, correct poor mechanics, strengthen weak muscles, or reverse pain from an untreated injury.
Fish oil can be part of your wise wellness plan. But, it should not be the whole plan.

Hey, Dr. Mike? “Can I get all of the Omega-3s I need from flaxseed, chia seeds, and walnuts instead of eating fish or taking fish oil, because I don’t like fish?”
Plant foods containing ALA are nutritious and worth including in a whole-food eating plan. Flaxseed, chia seeds, and walnuts provide fiber, minerals, polyphenols, and useful fats.
The issue and limitation is that ALA must be converted into EPA and DHA. That conversion is relatively inefficient. Therefore, eating ALA-rich foods does not always produce the same EPA and DHA blood index as eating fatty fish or using a more direct source of EPA/DHA.
That does not mean everyone needs a fish oil supplement. Someone who regularly eats fatty fish may already receive meaningful EPA and DHA through food choices.
For people who avoid fish, algae-derived omega-3 oil is the preferred vegan source because it can provide DHA and, depending on the product, EPA directly. Krill oil is sometimes mistakenly described as plant-based, but krill are small crustaceans. Krill oil is neither vegan nor appropriate for someone with a shellfish allergy.

Have very high triglycerides or complex cardiovascular disease.
Have liver disease.
Are pregnant or breastfeeding.
Have a fish allergy.
Stop and seek guidance if you develop new palpitations, rash, significant reflux, unusual bleeding, nosebleeds, or another concerning reaction.
